Now that you have filled out and submitted the Main Registration Form, you need to fill out and submit the Program Sign-Up Form.
Please be sure that the information you enter on the Program Sign-Up Form matches the information you entered on the Main Registration Form. If the information on the two forms do not match, the system will not be able to generate a username and password.
Please read through the following directions on how to fill out and submit the Program Sign-Up Form.
After reading and understanding the directions for filling out and submitting the Program Sign-Up Form, please click on the Program Sign-Up Form box at the bottom of the page. The Program Sign-Up Form will open in a new browser page so that you may refer back to this page if you have questions while filling it out.
Failure to read and follow the directions may result in your username and password being delayed or not being generated.
DO NOT fill out and submit the form multiple times. Doing so will result in your registration being delayed or denied.
The first screen you are going to get is going to look like the one to the left.
The first thing you will need to do is choose the name of the driving school you are registered with from the drop down box at the top of the form.
The box says: _Choose_Your_School
After you have chosen the name of the driving school you are registered with, you will next need to click on the " New " link at the botton of the form. This will open up the form for you to enter the information requested.
The second screen you will get looks like the one below. Just follow the instructions in the blue boxes to the right of the fill-in form. These instructions will NOT be on the form itself. Feel free to refer back to this page for the instructions. Please be careful to enter the correct information in the correct box.
Once you are done filling out the form, please click on the " Insert " link located at the bottom of the form. Once you click on the " Insert " link, the data you entered will be sent to the program system. You will NOT recieve any confirmation that the date was entered when clicking on the " Insert " link. The form will be cleared of all data. Just close your browser window to exit the page. The system will send you an e-mail indicating that it received your Program Sign-Up Form. It may take several minutes for you to receive the e-mail from the system. The e-mail will read:
Last Name, First Name has signed up for online drivers ed with your school (name of driving school) (DOT #). Phone contact: 000-000-0000 Email: (student's email address) StudentID: 0000 SchoolID: 000
If you are ready to fill out the Program Sign-Up Form, just click on the Program Sign-Up Form box below.
Once you are done filling our and submitting the Program Sign-Up Form, just close your browser window to exit this page.
Please remember that the system will NOT send you the username and password. The system will send the username and password to the driving school you are registered with.
